POS1355 THE REMITTING SERONEGATIVE SYMMETRICAL SYNOVITIS WITH PITTING EDEMA SYNDROME (RS3PE): REVIEW OF TEN YEARS AT A REFERENCE HOSPITAL

Background: The Remitting Seronegative Symmetrical Synovitis with Pitting Edema Syndrome (RS3PE) is a rare rheumatological disease, considered benign process. Objectives: This study aims to describe its clinical features and serological markers, also analyze possible association neoplasms. Methods: An observational retrospective was performed assess demographic characteristics of patients diagnosed from RS3PE at reference hospital amongst the Rheumatology Internal Medicine departments, 2010 2021. Results: Twenty-seven were included, mean age 82.74 y.o. (IC95% 80.45-85.04; range 66 93), 51.85% proportion males. Only 22.22% rural areas. All presented bilateral hand edema although some associated feet (40.74%) or morning stiffness (70.37%). Blood tests demonstrated anemia in 44.44% patients. Inflammatory markers elevated, such as C-Reactive Protein (29.23 mg/L, IC95% 18.17-40.29), erythrocyte-sedimentation rate (33.74 mm/hour, 24.22-43.26) fibrinogen (531.6 mg/dL, 482.91-580.30). few any autoimmune marker antinuclear antibodies (18.18%) rheumatoid factor (8.70%). X-ray screening realized 22 14 showed osteoarthritis radiologic presentation, 4 had radiological findings chondrocalcinosis one them both. patient bone erosion. Malignancy diagnosis only 29.63% (all negative). During follow-up two tumors detected (mean accumulated follow-up: 40.37 months, 26.70-54.04; 1 122) there adenocarcinoma primary but received low-dose corticosteroids, good rapid response all cases. Three treatment methotrexate (2) leflunomide (1). Conclusion: must be contemplated elderly presenting pitting articular symptoms. No specific biomarkers have been described, inflammatory reaction often found absence arthritis biomarkers. Rapid corticosteroids prevalent. neoplasms during follow-up. We describe four patients who presented with seronegative inflammatory peripheral polyarthritis with pitting edema. All of these patients had spontaneous resolution of their disease over 9-18 months. None of them developed erosions or relapse after prolonged follow-up. Recognition of the features described may allow for conservative therapy with less toxic drugs.
